Application forms

Instructions required to obtain qualification certificates with remote service :

Download the Qualification Certificate Application Form posted on the Agency 's website.
Fill in the sample accordingly and be sure to indicate the address where the finished certificate will be mailed;
You can digitally sign the application or put your signature as a picture or print, sign and then send us the scanned version;
Fill in the invoice form posted on the Agency's website and pay the amount to our bank account according to the subscribed invoice ;
Youmust send the completed certification , required documents and receipt paid in accordance with the invoice to the following e-mail address : stcw@mta.gov.ge
If you need to submit an additional document, you will receive a notification by e-mail and / or SMS;
The Agency will provide the finished documents to you by mail to the address specified in the application;
